Phoenix 1-A
Unmanned. First test of Jupiter 246-B with four SRBs, and Orion capsule.
Failure - Launch proceeded normally until 14 seconds prior to SRB seperation, at which point the vehicle exploded. Cause of the accident was determined to be a computer fault which led to an accidental firing of the range safety system. The launch escape system was activated and successfully pulled the unmanned Orion away from the launcher.
